This form is for uploading Letters of Reference for applicants to the Colorado State University Chemistry Department graduate program. If you received an email from our department requesting a letter, please use the first and last name of student as it appeared in that email.

Select one of the following file formats:

Your file will be renamed studentlastname.studentfirstname.yourlastname.ref.yyy (where yyy depends on the file format you chose) based on the information that you have entered. Some browsers don't seem to understand the JavaScript correctly and produce a filename of lastname.firstname.null.null.null. We will correct the filename if this happens to you.

When your file has successfully been uploaded you will receive a response page saying that your file was received.
